Graduate Loan Questionnaire

Our office will process a Federal Direct Unsubsidized Loan for any eligible student who submits a FAFSA using the 001444 school code. Students who decline part or all of that offer, or who gain additional eligibility, must complete GW’s Supplemental Graduate Loan Questionnaire to apply. Students applying for a Federal Direct Graduate PLUS or private alternative loan must also submit that form.
