Loan Applications and Forms
- 2023-2024 Supplemental Graduate Loan Questionnaire (PDF) – Used to request additional Federal Direct Unsubsidized Loan funding, and Federal Direct Graduate PLUS or private alternative loans. When completing this form please be as accurate as possible in determining if the course instruction will be offered at the Main Campus (Foggy Bottom/Mount Vernon/Corcoran), via Online Learning, or at an Off-Campus site. If you take classes at a different campus than indicated on this form, it may result in adjustments to your loans.
- 2023-2024 Graduate Change Form (PDF) – Used to reduce or cancel loans, rebudget or redistribute loans, or reinstate undisbursed Federal Direct Loans.
- 2023-2024 Verification of Outside Awards (PDF) - Submit only if awards are not displayed on GWeb.
- 2023-2024 Graduate Federal Work Study Request Form (PDF) – Used to request consideration for the Federal Work Study Program.
